2011-2012 Season Registration

Player registration for all levels EXCEPT HIP takes place online. This allows for a much more efficient registration process and saves our registrar a considerable amount of data entry time as well.  EVEN IF YOU PLAN TO COME TO ONE OF OUR OPEN HOUSE EVENTS YOU SHOULD COMPLETE YOUR REGISTRATION ONLINE FIRST AND BRING YOUR PAPER WORK WITH YOU!

  • Your fees include a $20 rink cleaning fee as well as a $20 jersey fee (for future jersey replacements)
  • Skaters will not be allowed to try out until all fees are paid and all forms are returned.
  • There will be a $30 charge for all checks returned due to insufficient funds!!

Payment for fees may be paid in full due by August 31, 2011 or a late payment fee fill be assessed. 

A partial payment option is also available.  Post dated checks in 1/4 total fee increments is required.  The checks will be dated and cashed on August 15th, September 15th, October 15th and November 15th.  If paying in installments, your registration with all post dated checks MUST BE RECEIVED ON OR BEFORE AUGUST 15, 2011.  If paying/registering after August 15th, full payment will be required.

NOTE: IT IS RECOMMENDED THAT YOU USE INTERNET EXPLORER (ON PC) OR SAFARI (ON MAC).  OTHER BROWSERS HAVE BEEN REPORTED TO HAVE PROBLEMS PROCESSING REGISTRATION FORMS.

All other levels begin your registration process by following the steps below.

Step 1: USA Hockey Registration

It is mandatory that all players register online with USA Hockey. The online registration process is simple and should take you only a few minutes to complete:

  1. Go to the USA Hockey Registration website
  2. Simply follow all of the prompts to register.  Make sure to use the full legal name of your player (no nicknames or shortened version of the name).
  3. You will need a credit card to make payment for your USA Hockey registration.  This cost covers your required membership in USA Hockey and WAHA).
  4. At the end of the on-line registration process you will be given a confirmation page with a bar code on it. PRINT A COPY OF THIS PAGE.  YOU WILL NEED TO TURN IT IN WITH THE REST OF YOUR REGISTRATION PACKAGE.

Step 2: Green Bay Area Youth Hockey Association (GBAYHA) Online Registration

Next you will need to register online with the Green Bay Area Youth Hockey Association.  To do this you will need the USA Hockey Player Confirmation Code that you received when you completed the USA Hockey online registration.

  1. Go to the GBAYHA registration web page
  2. Complete the registration process for each player.
  3. Print a copy of your GBAYHA registration confirmation and submit as part of your registration packet.

Step 3: Participation Agreement

You will need to read and complete the participation agreement form below.  This form needs to be submitted with the rest of your registration forms.

Step 4 - SCRIP Rebate Calculation (Required only for members who use SCRIP)

The form below will calculate your fees reduction from using SCRIP.  This form needs to be printed and returned with your registration materials.

IF YOU DO NOT USE THE SCRIP PROGRAM YOU MAY SKIP TO STEP 5.

Step 5: Submit Registration Forms and Fees to GBAYHA

Next you will need to return your registration information to our association. Please remember to include the following:

  • A printout of your USA Hockey registration confirmation page
  • A printout of your GBAYHA registration confirmation page
  • A signed copy of your participation agreement
  • A printout of your SCRIP rebate page from the SCRIP website (only required for members who use SCRIP)
  • A printout of your SCRIP calculation form (only required for members who use SCRIP)
  • Payment in full for player registration fees.
    Checks should be made payable to the Green Bay Area Youth Hockey Association. 
    The Green Bay Area Youth Hockey Association requires full payment at registration however we do offer an installment payment plan.  If you are making payment in 4 installments your checks must be dated to be cashed on, August 15th, September 15th, October 15th and November 15th.  If paying in installments, your registration with all post dated checks MUST BE RECEIVED ON OR BEFORE AUGUST 15, 2011.  If paying/registering after August 15th, payment in full is required.

  • A separate check for required credit deposit as follows:

This payment will be returned to you in full at the end of the hockey season if you have completed your required credits and service hours.

  • Each family (all skaters in GBAYHA except HIP) are required to volunteer an additional FOUR (4) hours at the U16 Tier II Nationals, hosted by GBAYHA and CCC on March 28th-April 1st, 2012.  For this reason each family will need to submit a separate Credit Deposit of $100 at registration.  After April 1st if you have completed your 4 volunteer hours at the U16 Tier 2 Nationals your check will be shredded.  Or, if you would like your check returned to you, include a self addressed stamped envelope with your registration.
  • A separate check for $50 (or $200 for second year Bantam, girls U14 or out of association skaters) for your jersey deposit. HIP, U8 and Squirt Developmental are exempt.  This payment will be returned to you near the end of the hockey season when you have returned your jersey in good condition.

You may also drop off your registration materials at the Cornerstone Community Center on our Open House Nights which will be held on July 25th, 2011 and August 10th, 2011 or you may place your registration materials in an envelope no larger than size 10 in the safe located in the GBAYHA office at the rink.

All registration forms and fees must be received at the ice arena no later than August 31, 2011 (August 15th if using installment payment plan).

Step 6: Sign Up For Credits on DIBS

Sign up for concession stand credits will take place online again this year.

NOTE: YOU WILL NOT BE ABLE TO SIGN UP FOR CREDITS UNTIL ALL FEES ARE PAID.  This includes your player registration fees and your credits payment.  After your fees have been paid, you will receive an email stating that you have been given access to the DIBS volunteer system which will allow you to select activities to fulfill your credits.

Remember, each family is required to work 4 credits per player.  U8 require 2 credits per player, Squirt Developmental 3 credits and HIP has no credit requirement.  Please be sure to sign up for the correct number of credits for your family.

To Sign up for credits:

  1. Login to the GBAYHA website by clicking on the login link at the top of the page.  If you do not already have a login for the site you will need to create one.
  2. After you have logged in, click on the DIBS tab on the top right side of the page.
  3. Click on the 2011-2012 Credits link in the Dibs screen.  This will give you access to the volunteer opportunities available.
